  3. Child’s Social Security card
  4. Certificate of Immunization Compliance (Form 121, original copy)
  5. Child’s Baptism certificate (Catholic families only)

Register Through FACTS

Once you’re ready to enroll, complete registration through our FACTS portal:

  1. Create or log in to your FACTS account — you’ll need a valid email address to get started
  2. Complete your child’s registration form and upload the required documents listed above
  3. Pay the registration fee to finalize your child’s spot
